Wellness Nurse Job Summary:
The Wellness Nurse promotes the provision of Resident care utilizing a team approach in concert with the Care Associates, Med Techs, and other members of the interdisciplinary team such as activities, dietary, and housekeeping. The Wellness Nurse provides direct care as well as directs and oversees care provided by the certified nursing assistant to ensure the Resident’s plan of care is provided within the policies, procedures, and practices of the community. In providing and overseeing care, a Wellness Nurse promotes the physical, personal, and emotional well-being of each Resident and strives to maintain the Residents at their maximum level of self-care and independence. The ultimate goal of quality Resident care is to enable each Resident, where possible, to return to the highest level of functioning, to attain a new stage of independence, or to at least prevent regression. An active approach to Resident care will be taken and not just passive maintenance.
Essential Functions:
The following essential functions are normal for this position. This list is not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
General Duties:
- Support the mission, vision, and goals of the organization, upholding and promoting company culture and vision.
- Provide direct nursing care services to Residents. Follow the individualized plan of care for all Residents.
- Perform Resident assessments as required by the state standards/ regulations and community policy, under the direction of the Wellness Leadership team.
- Observe Residents for changes in condition and report any changes immediately to the supervisor and/or the Director of Wellness. Provide timely interventions to change of condition and share this with care staff and Family as indicated.
- Document all pertinent/relevant observations, red flags, incidents, interventions, communication with the Resident’s responsible party, physician (as necessary), clinical procedures, and daily medication assistance/care throughout the shift as indicated and no later than by the end of each shift. Effectively pass the required information to the next shift in a timely manner and as indicated to the direct supervisor to ensure quality Resident care. Oversees the timely documentation by certified nursing assistant of services provided and red flags for their shift.
- Complete and maintain the current individualized care plan for each Resident. Provide interim documentation as required to Resident file.
- Attend all scheduled interdisciplinary Resident care meetings. Hold problem-solving stand-up communication or meeting with staff each shift.
- Check and record vital signs, weights, and blood sugars as assigned.
- Provide for or assist Residents with activities of daily living as needed, including bathing/showing according to schedule, oral care, toileting, dressing, etc. Perform restorative nursing techniques including proper positioning, ADL training, and Passive Range of Motion. Assist in providing appropriate information and behavioral prompts, cues, redirection, and affirming support to Residents in need. Support hydration programs and good dietary intake.
- Encourage Residents to participate in scheduled community activities aiding in getting to activities as needed. Assist in the planning and implementation of Resident activities as needed.
- Assist Residents with transfers and ambulation as directed, using proper body mechanics and behavioral techniques.
- Respond to urgent calls as needed in a timely manner.
- Always maintain restraint restraint-free environment. Support fall prevention programs for all Residents in need. This includes proactive intervention use, monitoring, and safety in the rooms and rounds.
- Help welcome, orient, and transfer Residents according to community procedure.
- Assist in post-mortem care as instructed.
- Communicate appropriate and timely information to Families as indicated. Report any Family concerns to the Supervisor and or Director of Wellness. Share with staff information they may need to know to improve care delivery in a timely manner.
- Oversee and manage medications delivery system to include timely order entry, receipt, and tracking as well as daily MAR documentation, narcotic count, and checking in medications in a timely manner as assigned to ensure Resident compliance to medication regime. Notify the pharmacy and supervisor of issues and seek to resolve them in a timely manner.
- Attend in-service classes and staff meetings. Orient new Associates. Lead and coach certified nursing assistants to improve performance.
- Obtain and demonstrate knowledge of the Discover Your Spark Lifestyle and Wellness philosophy and its inherent core values. Support and participate in Discover Your Spark (e.g., assist with feedback groups, assist in new initiatives, encourage Residents, and act as champions of fun).
Safety and Sanitation:
- Understand and follow safety and infection control policies and procedures.
- Follow established procedures in the use and disposal of personal protective equipment.
- Wear and/or use safety equipment and supplies (e.g., gait belt, back brace, mechanical lifts, etc.) when lifting or moving residents, as applicable to your position.
- Keep floors dry. Report spills immediately.
- Report all hazardous conditions and equipment problems to the supervisor immediately.
- Assist housekeeping in maintaining offices and common areas ensuring rooms are safe, comfortable, attractive, and orderly.
Training:

- Attends in-service and staff meetings and education programs.
- Continue to develop job-related knowledge, skills, and abilities by participating in continuing educational opportunities for personal growth and development.
Resident Rights:
- Promotes and protects residents’ rights, assists residents in making informed decisions, treats residents with dignity and respect, and supports residents’ independent expression, choice, and decision-making consistent with all pertinent laws regarding residents’ rights. Reports suspected abuse or neglect in accordance with applicable laws and regulations.
Other:
- Perform other related duties as assigned.
Desired Skills and Experience:
- Ability to work with the elderly in a courteous and friendly manner, demonstrating patience and compassion.
- Ability to perform duties with consideration for Residents’ rights at all times and demonstrate integrity and confidentiality in the care of Residents and in handling their health information.
- Knowledge of medical terminology, medical records management, medical supplies and equipment, and insurance forms. Experience with clinical and diagnostic procedures.
Required Education and Experience:
- High school diploma or equivalent (GED)
- Licensed Practical Nurse/Registered Nurse in the applicable state
- CPR, First Aide
- At least 1 year of related work experience

Must physically be able to lift up to 25 pounds unassisted. Must have normal eyesight or use corrective lenses. Must have three-dimensional vision and ability to judge distances and spatial relationships so as to see objects where and as they actually are. Must be able to recognize colors. Must be able to hear and distinguish between normal tones and be able to perceive the nature of sounds. Must be able to exchange ideas by means of the spoken word as well as engage in activities to convey detailed or spoken instructions to other workers accurately, loudly, and/or quickly. Must have normal sense of touch and smell.
Language Ability:
- Must possess excellent written and verbal communication skills utilizing the English language.
- Ability to read and interpret documents such as safety rules, operating/maintenance instructions, and procedure manuals.
- Ability to write routine reports and correspondence via print and email.
- Ability to effectively present information and respond to questions from managers, clients, customers, and the public.
Mathematical Skills:
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
- Ability to compute rate, ratio, and percent and to draw and interpret graphs.
Cognitive Demands:
- Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.
- Ability to deal with problems involving several concrete variables in standardized situations.
Computer Skills:
- Ability to use Microsoft Office Programs.
- Ability to use the Internet and online-based databases.

- Works primarily indoors in a climate-controlled setting.
