Qualifications of an ideal Assistant Wellness Director:
- CPR Certified
- Preferred 1+ year experience overseeing compliance in Pennsylvania Personal Care Home.
- LPN Certified
- Associate's degree or equivalent combination of education and experience
Assistant Wellness Director Job Summary:
An Assistant Wellness Director is responsible for overseeing and coordinating the care and services of Residents, promoting the effective utilization and monitoring of healthcare resources, and assisting in leading a multidisciplinary team to achieve desired clinical, emotional, and wellness outcomes.
- This temporary role has been established to enhance onsite leadership capacity and support day-to-day clinical operations.
- Support the mission, vision, and goals of the organization, upholding and promoting company culture and vision
- Establish and monitor goals and expectations for the Assisted Living and Memory Care departments
- Prepare departmental budgets, re-forecasts, and staffing plans for Wellness Director approval
- Participate in the admission and ongoing Resident assessment and care planning process
- Conduct clinical quality assurance audits including medication cart audits, medication pass observations, and comprehensive chart reviews
- Perform routine resident rounds to ensure safety, regulatory compliance, and adherence to individualized care plans
- Monitor secure medication storage for residents who self-administer medications and provide education as needed
- Inspect bed mobility and enabling devices to ensure proper installation and minimize safety risks
- Conduct daily Memory Care rounds and identify areas requiring intervention or follow-up
- Collaborate with therapy, hospice, home health, and other third-party providers to ensure recommendations are implemented and documented
- Facilitate communication between residents, families, providers, and the wellness team
- Review care plans and monitor services for the community’s most frail residents to ensure appropriate care delivery
- Identify, document, and correct areas of non-compliance and provide coaching to associates when necessary
